This practice aims to ensure the highest standard of medical care for our patients. It is not possible to undertake medical care without collecting and processing personal data and health data. Our policy is to only collect and record information about you that assists in your treatment.

TERMS & CONDITIONS 

Balbriggan family practice and its trusted third parties will:

Collect, use and retain personal and medical data about a patient in order to provide the highest standard of medical care. This data will be stored on a secure database which is fully encrypted.

For purposes of treatment, the administration staff may have to access your health data e.g. Reissue of prescriptions, recording information with other third-party medical providers, hospitals, consultants etc via Healthmail (encrypted email service) and post if required. If any health data is shared it will normally be limited to information related to a particular treatment and would not be a patients entire file.

Anonymise medical information if used for mandatory clinical research, audits and training of GP’s or other clinical staff.

Communicate with patients via SMS for appointment notifications, reminders and routine test results that are normal.

A patient can withdraw their consent for processing their personal data at any time.

DECLARATION OF CONSENT 

I understand the terms and conditions as outlined above and I am informed of Balbriggan Family Practice’s use of my personal and medical data. I thereby freely give my consent to the practice to process this data for the purposes of my ongoing health care and treatment.
